Gerald J. Ptak (Jerry) Born in Cleveland, OH April 10, 1941, age 73. Lived in Mentor, OH 19 years before moving to Heritage Hills in Clermont, FL. He passed away May 18, 2018 at home. Jerry was a volunteer firefighter and rescue worker, enjoyed boating, and loved his Harley, pool & yard. He loved Bridge, cards & chess. He had a good sense of humor. He worked for AT&T for 35 years in various departments. Survivors are his wife of 38 years, Betty (B.J.); children Dan, Scott & Jessica; ten grandchildren, one great-granddaughter; brother Tom (Cheryl) Vienna, VA; sisters Barb, Dale, Valerie & Terry (Lenny) Lange; beloved pets Cody, Erin & Jake. Proceeded in death by parents Joseph & Betty; sister Betty Rudd & granddaughter Heather White. A memorial service will be at 1:00 PM on Tuesday, June 3, 2014 followed by a gathering until 4:00 PM in the Clubhouse at Heritage Hills in Clermont. Reverend Dawn Fryman, Minister of Pastoral Care at First United Methodist Church of Clermont will officiate. In lieu of flowers donations to the Humane Society.
